I am writing to notify you that effective November 9, 2012 I will be closing my gastroenterology practice. I have published notice this closing in the newspaper over the past month.
Dr. Scott Corbett and Dr. Douglas Kuperman will become the record holders and will be happy to participate in your ongoing gastrointestinal care. They will have all the records available at their office, 2089 Hawthorne Street, Suite #200, Sarasota, FL 34239 and their office telephone number is (941) 365-6556. You may transfer your care to any other gastroenterologist or continue your care with them. They will attempt to mail a reminder notice when you are due for follow-up Colonoscopy, but if you do not receive notice, please call their office to schedule consultation.
I have tremendously enjoyed my practice of Gastroenterology in Sarasota and prior to that in St. Petersburg, Florida. It has been a privilege to care for many wonderful patients and their family members. I am saddened at needing to make a change in my practice, but for personal reasons must do so.
